請先下載 會員入會申請表，並請清楚填寫有關資料。
Download the membership application form, and fill in all the
requested data.

將填妥的會員表格電郵至本會電郵信箱 : application@hkpaperart.net
Send the completed membership application form to our e-mail
address: application@hkpaperart.net

3.
|
本會將於翌日由專人以電話聯絡已獲接受入會的會員。
Our staff will contact the accepted member by phone in the next day.

己獲接受入會的會員請於十個工作天內用以下方式繳交會費︰The fees are to be
paid by the member in 10 working days by one of the following methods ︰

請連同會費（現金或支票）親自或委託他人交往九龍觀塘創業街36號華基中心1711室（近牛頭角地鐵站）香港紙藝學會。
支票以「香港紙藝學會有限公司」或「HONG KONG PAPER ART
ASSOCIATION LTD」名義抬頭，並加劃線；或 請留空【抬頭人】一欄，以便工作人員當場代為填上本會的正確名稱；
Please send the
check or cash to Hong Kong Paper Art
Association Ltd., Rm. 1711, 36 Ricky Centre, Chong Yip St., Kwun
Tong, Kln., in personal or by an entrusted person.
The crossed check is of heading "HONG KONG PAPER ART
ASSOCIATION LTD". Or Leave the heading
blank, and let our staff fill in the name of our association.

Please pay the fees in Hongkong and
Shanghai Bank by cash or transfer (the account number: 814-014460-838, the
account name: Hong Kong Paper Art Association Ltd.), and send the bank slip
with the phone number and the name of the member to our office by e-mail （application@hkpaperart.net）or by fax (the facsimile number: 3422-8120) at
the same day.

本會將於一個月內郵寄會員証至會員提供的通訊地址。
Our association will send the membership card to the mailing address
of the member in a month.

如會員遺失本會會員證，請下載並填寫好補領會員證申請表後，連同行政費用港幣五十元正以以下方式繳交費用︰
If the member loses the membership card, please download and fill Re-Issue Membership Card Application Form , and send with administration fee HKD50 by one of the following
methods ︰

連同所需費用支票（支票以「香港紙藝學會有限公司」或「HONG KONG PAPER ART ASSOCIATION LTD」 名義抬頭，並加劃線；
支票背面寫上會員姓名、會員編號及聯絡電話號碼等資料；
寄達九龍觀塘創業街36號華基中心1711室「香港紙藝學會」會計部，並於信封面註明「補領會員證」字樣；
Please send the crossed check of heading "HONG KONG PAPER ART
ASSOCIATION LTD" to the account dept., Hong Kong Paper Art Association
Ltd., Rm. 1711, 36 Ricky Centre, Chong Yip St., Kwun
Tong, Kln., with “Re-issue Membership Card
